| Emergency Management Acknowledgement Form |
This form is used each time a mutual aid unit is requested to respond to your district and should be signed by department Chiefs, the one requesting the mutual aid and the one receiving the mutual aid. This form should then be forwarded to the department of Emergency Management. |

| FD-EMS agency list |
With the new CAD system the out-of-county agencies were given a new numbering scheme. An example of this is Chittenango FD used to be number 66 but is now 2705. This form is especially helpful for those departments that may request mutual aid units from one of the surrounding counties. |

Premise Info Sheet |
| This form is used to add Premise Information per Onondaga County 911 Emergency Communications General Order # 11-014 to a specific building/address. Such information can includes tactical information like sprinkler or standpipe information as well as key or lockbox information |

We try to standardize what the number in the response plan signifies to make it easier on those entering and maintaining the information. This form lists suggested naming conventions that can be used when creating/updating your response plans. |

Type File Worksheet |
This form shows the default response plans that 911 center personnel will link with the available event types. You can change the default response plan to another one by filling in the one you want used in the "Special" column. As an example if you wanted mutual aid on a fire alarm but not on a carbon monoxide alarm you might choose the response plan of "INVESTIGATION" to use instead of the default “ALARMS". |

Response Plan Form |
This is the most recent form that should be used when modifying or creating new response plans. The form has pop up windows that explain the type of information to be placed in each box.
